How do you enroll?
If you are currently diagnosed with diabetes or prediabetes
- If you are an existing member, and we identify you as having been diagnosed and/or treated for diabetes or prediabetes within the last 12 months, you will be automatically enrolled in the Diabetes Health Program. If you decide you would like to opt-out, you must do so with a UnitedHealthcare representative in the Risk & Benefits Management Department no later than March 31, 2013. You must complete the required Opt Out Form and return it to the UnitedHeatlhcare representative. You can contact one of the representatives at the Risk & Benefits Managerment Department at (561) 434-8580. Please note that if you opt out, you will not be allowed to enroll in the program until next open enrollment.
- If you are newly diagnosed or have not been insured with UnitedHealthcare for 12 months, you must register by using the paper registration form. Simply fill out the paper registration form and then have your doctor sign the form to verify your diagnosis. You or your doctor must fax the signed form to HealthInsight.
If you think you may have diabetes or prediabetes
- Make an appointment with your doctor to have a fasting blood sugar or A1c test. If your test indicates that your fasting blood sugar is 100 mg/dL or greater or your A1c test is 5.7% or higher, you are eligible to join. Simply fill out the paper registration form and then have your doctor sign the form to verify your diagnosis. You or your doctor must fax the signed form to HealthInsight.
