New for 2017! For those living with Diabetes, the new Personal Support – Diabetes program provides enhanced resources as part of your existing benefits.

Click “Get Started” below to visit the Personalized Support – Diabetes website and sign up. Be sure to have your insurance ID card ready.

After logging in, follow the simple 3 step registration process to get started:

  1. Confirm your mailing address to receive your complimentary Welcome Kit, which includes a water bottle, reusable lunch tote, cutting mat, resistance bands (and more)
  2. Download the OneDrop mobile app to track your blood glucose levels, medications, food and activity all in one place
  3. Set goals to keep you motivated and on track

You will also receive:

  • Online access to your Diabetes Health Plan scorecard to track your health actions
  • Personalized online resources
  • Access to a support team, including wellness coaches and nurses

For those living with Prediabetes or are At-Risk for diabetes:

  • Personalized online resources
  • Online access to your Diabetes Health Plan scorecard to track your progress
  • Access to a Wellness Coach and Nurse via Chat or by Phone in the mobile app
